    Please read these terms carefully before scheduling your consultation or receiving treatments at Quartz Aesthetic Clinic. By booking or using this website, you agree to these clinical and practice policies.

    Overview & Practice Scope

    Welcome to Quartz Aesthetic Clinic. As a Registered Nurse governed by the College of Nurses of Ontario (CNO), I provide professional medical aesthetic treatments and clinical wellness consultations. By accessing https://quartz-clinic.com or reserving an appointment, you acknowledge that you have read, understood, and agreed to be bound by these Terms of Service.

    Consultations & Treatment Eligibility

    Age RequirementYou must be at least 18 years of age (or the legal age of majority in Ontario) to receive medical aesthetic procedures independently.
    Medical DisclosureYou agree to provide complete, honest, and accurate medical history during your consultation, including all pre-existing health conditions, prescription medications, allergies, and past aesthetic treatments.
    Candidacy AssessmentI reserve the right to decline or defer treatment if I determine during evaluation that a procedure is clinically unsafe or unsuitable for you.

    Booking & Payment Terms

    Transparent PricingAll treatment prices are listed in Canadian Dollars (CAD) and are subject to applicable taxes (13% HST).
    Card on FileA valid credit card is required to hold your online appointment reservation. Your card will not be charged at the time of booking.
    Payment CompletionFull payment for all medical aesthetic services is due immediately following the completion of your session via Visa, Mastercard, Debit, Cash, or e-Transfer.

    Cancellation & Attendance Policies

    24-Hour NoticeI kindly request at least 24 hours notice for any appointment cancellation or rescheduling to allow other patients access to open time slots.
    Late Arrival PolicyIf you arrive more than 10 to 15 minutes late, your appointment scope may be modified or rescheduled to ensure the next patient's session remains unhurried.
    No-Show TermsRepeated missed appointments without advance notice may result in a non-refundable deposit requirement for future reservations.

    Medical Disclaimer & Aesthetic Outcomes

    Individual VarianceHuman anatomy and skin biology vary significantly. While every procedure is administered with clinical precision and hospital-grade care, individual aesthetic results cannot be guaranteed.
    Educational ContentInformation provided on this website is for general educational purposes and does not constitute formal medical diagnosis until a 1-on-1 consultation is completed.
    Aftercare AdherenceYou acknowledge that optimal healing and aesthetic outcomes depend strictly on following the post-treatment aftercare instructions provided to you.

    Limitation of Liability

    To the maximum extent permitted by Ontario law, Quartz Aesthetic Clinic and Nurse Moti shall not be liable for any indirect, incidental, or consequential damages arising from website usage or non-compliance with post-treatment aftercare protocols. You agree to indemnify and hold harmless the clinic against claims resulting from inaccurate medical information provided by you during consultation.

    Intellectual Property & Digital Rights

    All content, graphics, logos, branding, photography, and text on https://quartz-clinic.com are the exclusive intellectual property of Quartz Aesthetic Clinic. Reproduction, copying, or unauthorized commercial use of any website asset without prior written consent is strictly prohibited.

    Governing Law & Dispute Resolution

    These Terms of Service are governed by and construed in accordance with the laws of the Province of Ontario and the federal laws of Canada applicable therein. Any legal proceeding or dispute arising under these terms shall be subject to the exclusive jurisdiction of the courts of Ontario.

    Cost & FrequencyMessage frequency varies based on your appointment activity. Message and data rates may apply.

    Carrier LiabilityMobile carriers (AT&T, T-Mobile, Verizon, Rogers, Bell, Telus, etc.) are not liable for delayed or undelivered messages.
